Payment Authorization

Directions:
 
1. Download and fill out the PTA Payment Authorization form
 
2. After filling the form out, the document needs to be saved to your computer before you email it out, or printed as a PDF to your desktop.  If you email the form before saving/printing as PDF to your desktop it will erase everything you have typed in and I will receive a blank form.  
 
3. MAKE SURE YOU SIGN (Until we are back at school, a virtual signature is ok) and date
 
4. Scan receipts or take a photo and attach
 
5. Email the form and receipts to  treasurer@alisalpta.org
 
If you prefer the old method of printing the form and hand filling it in, and emailing to treasurer@alisalpta.org with your receipts attached, that is also fine.
